This Common Age-Related Change May Raise Dementia Risk by 42%
Your muscles may matter for brain health in ways you didn’t expect.

Muscle strength doesn't always get the same attention. But a new analysis suggests1 it may deserve a bigger spot in the conversation. Researchers looked at how muscle loss and excess body fat relate to dementia risk, and the results point to an interesting difference between the two. In particular, losing muscle was consistently linked to higher dementia risk, while the relationship with body weight changed depending on age.About the study
Researchers conducted a systematic review and meta-analysis, meaning they gathered and combined results from existing studies to look for a larger pattern.
They included 83 cohort studies that examined body composition and dementia risk. Fourteen studies looked at sarcopenia (the age-related loss of muscle mass and strength) while 72 studies focused on obesity, and 4 examined sarcopenic obesity, a combination of low muscle mass and excess body fat.
The researchers searched four major medical databases for studies available through February 2026.
They were particularly interested in whether muscle and fat were linked to dementia in different ways, since BMI only tells you how much you weigh for your height. It can't tell whether that weight comes from muscle or body fat. That broader look at body composition revealed some notable differences.
Muscle loss was linked to higher dementia risk
People with sarcopenia had a 42% higher risk of all-cause dementia than people without it. The association was seen in both middle-aged and older adults.
The connection was even stronger for vascular dementia, which is caused by damage to blood vessels in the brain. Sarcopenia was associated with a 70% higher risk of this type of dementia.
Obesity had a more complicated relationship with dementia. Among adults younger than 65, obesity was linked to a modestly higher dementia risk. But among adults 65 and older, obesity was associated with a lower risk.
That doesn't mean extra weight protects the brain.
The researchers point to reverse causation as one possible explanation. Unintentional weight loss can happen during the early stages of dementia, meaning lower body weight later in life may sometimes be a result of the disease rather than a cause.
The analysis found no significant overall association between sarcopenic obesity and dementia, although that result is based on only four studies.
Why muscle may matter for the brain
Muscle does more than help you move. It's also metabolically active tissue that communicates with other parts of the body.
When muscles contract, they release signaling molecules called myokines, some of which can influence brain function. Maintaining muscle also makes it easier to stay physically active, which supports metabolic health and blood flow to the brain.
There's a simpler explanation, too. Losing muscle can make everyday movement harder, which may lead to less activity over time. That can affect several factors associated with brain health, including blood sugar regulation and cardiovascular health.
This may help explain why muscle health showed a more consistent relationship with dementia risk than obesity did in this analysis.

This study doesn't prove that maintaining muscle prevents dementia, and body weight still plays a role in overall health. But it adds to the evidence that muscle deserves attention as we think about aging and brain health.
